Adult Outpatient (AOP)
Program summary:The Adult Outpatient Program (AOP) provides ongoing mental health services to adults (18-64) who are experiencing psychiatric concerns. Services may include individual, family and group therapy, case management, rehabilitation services, peer support, and medication support. The goal is to stabilize clients, make them more independent, and acquire healthier coping skills to function better socially, emotionally and behaviorally within the home and in the community.
Program Enrollment Eligibility:Young adults and older adults, ages 18-64, presenting with emotional disturbances related to a DSM diagnosis.
Program Services:Adults in the Adult Outpatient Program (AOP) will find the following are potential examples of mental health services and supports clients may receive:
- Individual psychotherapy with the use of evidenced-based practices
- Group counseling
Medication evaluation and medication support services - Case management
- Linkage to physical health care services
- Linkage to employment, vocational training and/or volunteer opportunities
- Dedicated professionals committed to your success in accomplishing goals that are important to your health, well-being, safety and stability
Adult Field Capable Support Services/Resilience, Recovery, Reintegration (FCCS/RRR)
Program summary:Recovery, Resilience and Reintegration (RRR) Services are specialized clinical services delivered by a multi-disciplinary treatment team of mental health professionals and paraprofessionals specifically trained to work with adult populations. Some RRR clients may be unwilling to receive services in traditional mental health settings due to the stigma associated with mental illness. Services will be provided in locations in the community, such as the offices of a primary care physician or in their homes. RRR includes therapy, rehabilitation services, medication management and medication support, housing services, outreach and engagement screenings and assessments to determine the level of functioning and impairment, individual, dyadic, family and group psychotherapy, crisis intervention, family education and support, linkage and case management.
Program Enrollment Eligibility:This program is for clients ages 18-64 with a need for moderate intensity mental health services. Clients are referred by the community at large, including: Local Mental Health Clinics, Primary Care Settings, Homeless Shelters and Older Adult Residences, Hospitals, Religious Organizations and Senior Centers.
Program Services:The RRR services may be clinic-based, home-based, or community-based. They include collaboration with primary care services, peer supports, and supportive employment assistance. Clinic-based services will often utilize cognitive-behavioral therapy (CBT), dialectical behavioral therapy (DBT), and trauma services. Services are intended to increase well-being and reintegration. Some of the specific services may include:
- Outreach and Engagement
- Individual psychotherapy with the use of evidenced-based practices
- Group counseling
- Medication evaluation and medication support services
- Case management
- Linkage to physical health care services
- Home or community visits to increase accessibility and to strengthen a client’s network of relationships & decrease social isolation.
- Case Management—linkage and referral
- Benefits establishment services
Wellness Center
Program summary:The purpose of the Wellness Center is based upon the principles of Mental Health Recovery, which support values of hope, choice, respect, cultural sensitivity, achieving meaningful roles, self-determination, acceptance, and healing. Wellness Center programs and activities are directed towards empowerment and the development of self-efficacy and increased community functioning. This is executed in the forms of groups, peer support, and various resources.
Program Enrollment Eligibility:Young adults and older adults, ages 18-64, presenting with emotional disturbances related to a DSM diagnosis yet are stabilized (with or without medication support). The Wellness Center program would be focused primarily on medication-management services, enhancement of self-efficacy, and life planning.
Program Services:Services in the Wellness Center may include:
- Medication management and support services
- Limited counseling services
- Self-help and self-expression groups
- Case management
- Peer support
- Healthy living groups and interventions
- Linkage Services to essential resources in the community to further recovery
